1. SWEEPSTAKES PERIOD:
Starts every Wednesday at 6:00:PM Arizona Time and ends on the immediate following Saturday at 11:59:59 PM Arizona Time (the “Sweepstakes Period”).
2. ELIGIBILITY:
Several ways to enter. To enter and be eligible to win a prize, you must be a legal resident of the United States, or Washington, D.C. (excluding residents of Puerto Rico, the U.S. Virgin Islands and other U.S. territories and possessions, and where prohibited by law), be living in the greater Phoenix, AZ or surrounding cities or ability to pick up prize in person, and have a valid email address. Entrants must be 18 years or older at the time of the drawing. Persons in any of the following categories are NOT eligible to participate or win a prize: (a) any person who is a director, officer, agent, representative, or employee of Arizona Small Dog Rescue (“Sponsor”), or anyone who is directly involved in the planning or execution of the Sweepstakes; (b) persons who are or purport to be immediate family members (defined as biological or step-mother/ father, sister, brother, daughter, son, dependent or spouse of any of the foregoing) of any person in any of the preceding categories, regardless of where they live, or who reside in the same household as any person in any of the preceding categories; (c) any person who is a director, officer, agent, representative, or employee of supplying prize organization (“Prize Sponsor”); and (d)persons who are or purport to be immediate family members of “Prize Sponsor” (defined as biological or step-mother/ father, sister, brother, daughter, son, dependent or spouse of any of the foregoing) of any person in any of the preceding categories, regardless of where they live, or who reside in the same household as any person in any of the preceding categories. Sweepstakes is subject to all applicable federal, state and local laws and regulations.
3. HOW TO ENTER:
NO PURCHASE NECESSARY (SEE RULE #4 FOR DETAILS). Entries can be made via a donation to PayPal, or via a donation to Venmo(ArizonaSmallDogRescue), with the word “SWEEP” in the note or comment section, or by filling out a free sweepstakes entry form in person at the shelter location when we are open, no purchase necessary See Rule #4 for details on how to enter without a donation.
Sponsor may, from time to time by various methods, offer opportunities to receive a Sweepstakes entry online at their discretion.
Sweepstakes Entries via donation: One (1) sweepstakes entry will be given for every $5.00 donation.
- Five Dollar Donation ($5.00) = 1 Sweepstakes entry
- Ten Dollar Donation ($10.00) = 2 Sweepstakes entries
- Fifteen Dollar Donation ($15.00) = 3 Sweepstakes entries
- Twenty Dollar Donation ($20.00) = 4 Sweepstakes entries
- Twenty-Five Dollar Donation ($25.00) = 5 Sweepstakes entries
4. ALTERNATE METHOD OF ENTRY:
To enter without purchase and without making a donation, visit Arizona Small Dog Rescue located at 1102 W Hatcher Rd., Phoenix AZ 85021 between the hours of 11am and 6pm. Ask our shelter staff for a Sweepstake Entry Form. You will be required to provide your full name, complete address, phone number (with area code), date of birth, and email address. For each form received, you will receive one (1) entry into the Sweepstakes. Forms must be received no later than 6pm on Saturday of Sweepstakes closing.
One (1) entry per day, per household allowed.
Entries submitted by form must be submitted with legible writing. All entries become property of the Sponsor and will not be returned.
5. RANDOM SWEEPSTAKES DRAW:
Every Sunday, at approximately 5:00 PM Arizona Time, the winner will be selected by using a random name wheel generator online (www.wheelofnames.com) for the Sweepstake Prize, which will be recorded and posted onto our Facebook page. (See Rule #6 for details) The odds of winning a prize depend on the total number of eligible entries received during the Sweepstakes Period. Potential Winner will be subject to verification of eligibility and compliance with these Official Rules.
6. SWEEPSTAKES PRIZE:
The Winner will receive the Sweepstake Prize posted on our Facebook page the Wednesday prior to the name draw. Total approximate retail value (“ARV”) of Prize will be included in Prize post. Sweepstake Prizes and ARV will vary by week.
*Acceptance and use of the prize is subject to the winner accepting the Terms and Conditions that apply to gift card(s) which may be included in prize package.
Any difference between the stated approximate retail value and the actual value of the prize will not be awarded.
The Prize is not transferable, or redeemable for cash, and must be accepted as awarded, with no substitutions of any kind, except by the Sponsor, who reserves the right to substitute a prize (or portion thereof) with one of comparable or greater value, at its sole discretion.
All federal, state and local taxes (including income and withholding taxes), if any, as well as any other costs and expenses associated with prize acceptance and use not specified herein as being provided are the Winner’s responsibility, regardless of whether a prize is used in whole or in part. All prize details are at the Sponsor’s sole discretion.
7. HOW TO CLAIM YOUR PRIZE:
Winners will be notified via email to the email address they entered the Sweepstakes with within five (5) days following the winner selection. Arizona Small Dog Rescue shall have no liability for a winner’s failure to receive notices due to winners’ spam, junk e-mail or other security settings or for winners’ provision of incorrect or otherwise non-functioning contact information. If the selected winner cannot be contacted, is ineligible, fails to claim the prize within 72 hours from the time award notification was sent, or fails to pick up prize in a timely matter, prize may be forfeited and an alternate winner selected.
Sponsor may select up to three (3) alternate prize winners; thereafter, if a potential prize winner cannot be verified in accordance with these Official Rules, the prize will not be awarded. Potential Winners will be required to furnish proof of identification.
The receipt by winner of the prize offered in this Sweepstakes is conditioned upon compliance with any and all federal and state laws and regulations. ANY VIOLATION OF THESE OFFICIAL RULES BY ANY WINNER (AT SPONSOR’S SOLE DISCRETION) WILL RESULT IN SUCH WINNER’S DISQUALIFICATION AS WINNER OF THE SWEEPSTAKES AND ALL PRIVILEGES AS WINNER WILL BE IMMEDIATELY TERMINATED.
8. RIGHTS GRANTED BY YOU:
By entering this content you understand that Arizona Small Dog Rescue, anyone acting on behalf of Arizona Small Dog Rescue, or its respective licensees, successors and assigns will have the right, where permitted by law, without any further notice, review or consent to print, publish, broadcast, distribute, and use, worldwide in any media now known or hereafter in perpetuity and throughout the World, your entry, including, without limitation, the entry and winner’s name, portrait, picture, voice, likeness, image or statements about the Sweepstakes, and biographical information as news, publicity or information and for trade, advertising, public relations and promotional purposes without any further compensation.
9. TERMS:
Arizona Small Dog Rescue reserves the right, in its sole discretion to cancel, terminate, modify or suspend the Sweepstakes should (in its sole discretion) a virus, bugs, non-authorized human intervention, fraud or other causes beyond its control corrupt or affect the administration, security, fairness or proper conduct of the Sweepstakes. In such case, Arizona Small Dog Rescue may select the recipients from all eligible entries received prior to and/or after (if appropriate) the action taken by Arizona Small Dog Rescue. Arizona Small Dog Rescue reserves the right at its sole discretion to disqualify any individual who tampers or attempts to tamper with the entry process or the operation of the Sweepstakes or website or violates these Terms & Conditions.
Any attempt by an entrant to deliberately damage any web site or undermine the legitimate operation of the sweepstakes may be a violation of criminal and civil laws and should such an attempt be made, Arizona Small Dog Rescue reserves the right to seek damages from any such person to the fullest extent permitted by law.

10. LIMITATION OF LIABILITY:
By entering you agree to release and hold harmless Arizona Small Dog Rescue and its subsidiaries, affiliates, advertising and promotion agencies, partners, representatives, agents, successors, assigns, employees, officers and directors from any liability, illness, injury, death, loss, litigation, claim or damage that may occur, directly or indirectly, whether caused by negligence or not, from (i) such entrant’s participation in the sweepstakes and/or his/her acceptance, possession, use, or misuse of any prize or any portion thereof, (ii) technical failures of any kind, including but not limited to the malfunctioning of any computer, cable, network, hardware or software; (iii) the unavailability or inaccessibility of any transmissions or telephone or Internet service; (iv) unauthorized human intervention in any part of the entry process or the Promotion; (v) electronic or human error which may occur in the administration of the Promotion or the processing of entries.
11. DISPUTES:
THIS SWEEPSTAKES IS GOVERNED BY THE LAWS OF United States of America AND State of Arizona, WITHOUT RESPECT TO CONFLICT OF LAW DOCTRINES. As a condition of participating in this Sweepstakes, participant agrees that any and all disputes which cannot be resolved between the parties, and causes of action arising out of or connected with this Sweepstakes, shall be resolved individually, without resort to any form of class action, exclusively before a court located in AZ having jurisdiction. Further, in any such dispute, under no circumstances will participant be permitted to obtain awards for, and hereby waives all rights to claim punitive, incidental, or consequential damages, including reasonable attorneys’ fees, other than participant’s actual out-of-pocket expenses (i.e. costs associated with entering this Sweepstakes), and participant further waives all rights to have damages multiplied or increased.
